Nurse-Family Partnership (NFP) Nurse Manager – Nurse-Family Partnership, a maternal and early childhood health program, fosters long-term success for first-time moms, their babies and society. The NFP program introduces vulnerable first-time parents to caring maternal and child health nurses. This program allows nurses to deliver life changing educational and social support in the homes of the first-time moms. Bilingual Patient Navigator - The YW’s Women’s Health (WH) program is seeking a Bilingual Patient Navigator. This individual is responsible to ensure eligible clients are guided appropriately through health care systems to receive the offered mammogram and breast health services. In addition to supporting and offering client services, the Patient Navigator will be an advocate for our program and deliver the program message through community awareness.
